Transition Program

 
The purpose of the Transition Program is to develop a new pattern of services for special education students during their last eighteen months of school. These new services will lead to the students becoming competitively employed or enrolled in a career-entry training program following their exit from high school. Students receive job placement and training services, job-related instruction, independent living skill training, and assistance with transition planning.
 
The Transiton Program is a year-round activity. There are at least three key service providers who will become a part of the Transition Program team and are responsible for the delivery of these year-round services. These service providers are:
  • Teacher Coordinator
  • Transition Specialist
  • Vocational Rehabilitation Counselor

In general, the responsibilities of the school (Teacher/Coordinator & Transition Specialist) include:
  • Recruiting students
  • Assessing students with the VR counselor
  • Developing IEPs coordinated with IWRPs
  • Teaching targeted competencies
  • Developing job sites
  • Supervising students on jobs
  • Crisis intervention
  • Independent Living Skill training
  • Creating systems change

The responsibilities of the VR Counselor include:
  • Providing program leadership
  • Establishing student eligibility
  • Developing IWRPs coordnated with IEPs
